Hi,
we have integrated our website to Hosted page in our case
(Bssi.chargevault.com) , my question is,
How is it possible to Charge a customer a plan with addons and
setup fee in that way ?

Setup fee is automatically part of the pricing plan (assuming you have your plans config'd with a setup fee). The hosted pages are designed to get basic revenue models up and running quickly and don't include some advanced features. Specifically, the hosted pages do not include support for tracked item quantity settings. Tracked item quantities must be set via the API (or manually via the admin GUI).
